Performance Evaluations

Performance evaluations serve as a constructive tool to identify and review performance against university-defined core expectations, as well as specific job responsibilities. In addition, performance evaluations may be tied to performance-based salary increases.

Per SPP 309-01 and SPP 808, classified, university staff and non-faculty administrators must receive an annual performance evaluation. Performance evaluations for student workers and Staff PRNs are optional.

Performance Evaluation Process

Current performance review period is from July 1, 2019 through June 30, 2020. All staff who worked during this time period, regardless of funding source, both part-time and full-time, must have an evaluation completed. This only applies to staff and does not apply to student workers, RAs, TAs, GAs, or GSAs.

Performance evaluations are to be completed thoughtfully and include the following elements:

  1. An evaluation of the employee's performance against the university core expectations as they relate to the employee's job
  2. A brief narrative summary of performance, accomplishments, and/or deficiencies in the evaluation period, focusing on the employee's specific job responsibilities
  3. An overall rating based on a five-point rating scale:
    • Rating level 1—Fails to meet performance expectations
    • Rating level 2—Inconsistently fulfills performance expectations
    • Rating level 3—Performance expectations fulfilled
    • Rating level 4—Frequently exceeds performance expectations
    • Rating level 5—Consistently exceeds performance expectation
  4. A brief summary of job-related goals for the upcoming evaluation period
  5. A brief summary of individual development opportunities for the upcoming evaluation period

Self-evaluations are voluntary but are beneficial for both the employee and the supervisor.

Staff responsibilities:

  1. Submit self-evaluations to their respective manager(s).

Supervisor responsibilities:

  1. Complete applicable performance evaluation form and review the Performance Development Plan.
  2. Provide the completed evaluation to staff 24 hours prior to the face-to-face (Zoom) performance review meeting.
  3. Meet with staff face-to-face (Zoom) to conduct performance reviews and discuss development plans (providing the completed evaluation to staff 24 hours prior to the actual review).
  4. Submit fully signed staff evaluations and any other related forms such as self-evaluations to Alyona Pochert at alyona.pochert@asu.edu (fully signed electronic copies of Staff/Manager Evaluations and Professional Development Plan forms).
    • When submitting the forms, please use the naming format listed below.


  • Performance evaluation form (choose one):
  • Performance Development Plan (DPD) form - Must be reviewed and updated on an annual basis (CHS policy). PDP is meant to aid an employee and supervisor with the employee’s development on a 1-5 year trajectory and align with the overall performance evaluation. 
  • Performance Improvement Plan form - Required when an employee receives a rating of 2 or 1 on their annual performance evaluation, or on as-needed basis. The Performance Improvement Plan must be reviewed and approved by CHS HR prior to being presented to your employee. Contact Alyona Pochert at alyona.pochert@asu.edu

Please use the following naming formats when you submit the forms to CHS HR:

  • Last Name, First Name_2020_Performance Eval
  • Last Name, First Name_2020_Self Eval
  • Last Name, First Name_2020_PDP

While supervisors may choose to use decimal ratings in the review itself, ratings must be submitted as whole numbers to OHR and PeopleSoft (e.g. 3.4 = 3.0, 3.5=4.0 ).

CHS HR responsibilities

  • Enter staff performance review ratings into PeopleSoft.
  • Mail a hard copy of evaluations to ASU's Office of Human Resources (OHR).
