Annual Evaluation Forms

The Annual Review 2020 is based on contributions over a 3-year review period (2018-2020) with particular attention to the last calendar year.

Tenure Track Faculty

There are four (4) forms for the tenure track faculty to complete unless you have administrative time in your workload agreement. If that is the case you will also need to complete the Administrator Annual Evaluation form.

Non-Tenure Track Faculty (NTE)

The non-tenure faculty will complete three (3) forms unless you have research and/or administrative time in your workload agreement. If that is the case you will also need to complete the Research form and the Administrator Annual Evaluation form.

Additional notes for committees

  • Faculty Feedback Form – to be completed by the Sub-committee lead, committee chair and Associate or Assistant Deans of Faculty Success
  • Process Form – provides guidance to the committees reviewing TT, NTE or Clinical Faculty Packages.
  • Each form includes instructions – please read carefully as some parts are for ‘reviewers only’ to complete.
  • All faculty are to submit their CV along with their annual evaluation forms. 
  • Answers need to be concise and detailed as there is a character limit. 
  • Please do not repeat information that is on your CV, rather your text should provide the reviewers with additional context and relevance.
  • For faculty with administrative effort, that component of the annual review will be conducted separately.

Tenure-Track Faculty

Tenure-Track Faculty (Archived Unit and College Criteria)

The CHS reorganization was effective 10/1/18. P&T candidates who completed their probationary review as of the 2018-2019 cycle may choose to use the previous CHS unit criteria (e.g., the P&T criteria for SHCD, SNHP, SHS or BMI) and the college criteria.

Clinical Track Faculty
